Our policies and information

Please take a few minutes to read through our policies and additional useful information


Payment is due at the time of service.
We accept: Cash, Debit, Visa, MasterCard


Two business days notice is requested for all cancellations as a courtesy to our staff and patients awaiting available appointments.
Phone our office directly and speak with the office personnel to avoid any cancellation fees.


The insurance policy is a contract between the insurer and the insured (the patient). Although we make every effort to help you understand your insurance policy, we are not liable for any amounts not covered by your plan.
It is the patient’s responsibility to advise us of any changes to the insurance policy.
Your wallet ID card enables us to send your electronic claim to your insurance company with the correct policy and certificate number. It does not give us any access to your policy information.
